Lung Cancer Awareness License Plate

To benefit Lung Cancer Awareness and Research through the American Lung Association of North Carolina

The American Lung Association of North Carolina invites you to purchase a Lung Cancer Awareness license plate. We are well on our way to selling the required 300 plates necessary for our plate to be released by the North Carolina Department of Motor Vehicles. Our goal is to have all 300 plates sold by December 31, 2006.

Lung cancer is the leading cause of cancer deaths in both men and women in North Carolina – we must find a cure for this deadly disease.

Cost

(Please note the fees are in addition to the standard license plate fees)

Lung Cancer Awareness License Plate: $15

Lung Cancer Awareness Personalized License Plate: $45*

* Four (4) spaces are allowed on the Lung Cancer Awareness Personalized Plate. May be a combination of letters and numbers, but cannot be numbers only. Choice cannot conflict with another class of license plate.

How to Purchase

To order a license plate, please download the the license application form below.  Send the completed application along with check or money order to: American Lung Association of NC, 3801 Lake Boone Trail, Ste 190, Raleigh, NC 27607. 

You may pay for the license plate online via credit card but you must still download and complete the license application form.
